Our team at Sunset Periodontics & Implant Dentistry extends our warmest wishes for your and your family’s ongoing health and safety. We are pleased to offer you a comprehensive examination with Dr. Mize. Based on his professional assessment, Dr. Mize will determine the necessity of utilizing either 3D Radiographs (CBCT) or Full Mouth X-rays (FMX) for your diagnosis. Please note, this exclusive offer covers the cost of the initial X-ray selected by Dr. Mize. However, should your diagnosis require both types of X-rays for a complete evaluation, an additional charge will apply for the second X-ray.
- A $50 deposit will be required for appointment booking. And you will pay the remainder ($88) on the day of your initial consultation.
- To transfer the x-ray to another dental office, we will ask you to pay the full cost of the x-ray.
- We are able to file claims with your dental insurance provider as a courtesy for your reimbursement.
- We are able to file claims with all dental insurance except for Medicare and Medicaid.
